CLI commands

All 23 commands in context-vault 4.0.0, with the descriptions from the CLI's own help. Run context-vault --help for the full list and context-vault <command> --help for flags.

`setup`	Register the MCP server with AI tools
`mcp`	MCP stdio server: what your client launches
`doctor`	Diagnose the install: db and vault paths, engine health
`status`	Print vault status: entry counts, health, resolved paths
`stats`	Print vault statistics as JSON
`reindex`	Rebuild the SQLite index from the markdown vault
`vault`	Manage vaults in the ~/.context-mcp/vaults.json registry
`schema`	Inspect or scaffold the per-vault taxonomy (vault.toml)
